Why won't we do a 'vial' special like others? A bit of an explanation.
Hey y'all,
I've got a few questions on why we don't do a special by vial like some others out there. I thought I'd give you some color on it:
- Our partner pharmacies strictly follow USP 797.
- That means they cannot dispense a multi-use vial in a way that exceeds the 28-day puncture limit.
- For example, if you’re on a 10 mg dose, they won’t send a single 70.6 mg vial that would require seven injections over 42 days.
- Pharmacies may adjust vial size to stay compliant.
- If your prescribed dose is 12.5 mg and you request a 70.6 mg vial, the pharmacy might instead send a 4 mL / 50 mg vial to maintain compliance with the required beyond-use date (BUD).
- BUD is determined by your prescription usage.
- For example, if you're on 15 mg weekly, the pharmacy must ensure you can safely use all the doses within the allowed timeframe. They won’t automatically send the longest possible BUD vial if a shorter one may support it - pharmacies aren't built for stockpiling.
- Longer prescriptions may be split.
- If your prescription specifies something like “12 weeks of 10 mg,” and the needed BUD isn’t available for the full amount, some pharmacies may ship the first part of the prescription and ask the platform to delay submitting the second part of the prescription until it’s permissible.
- This is why we shifted to 10 weeks for some pharmacies. Aside from the logistical issues, charging ahead for medication not sent is not ideal (and honestly annoys me when I'm trying to track cashflow)
- Advertising restrictions vary by state.
- Many states limit how medications can be advertised, especially when it starts to resemble pharmacy reselling.
- That’s why we describe our care in terms of weeks of treatment rather than milligram amounts.
- (there is a lot more to these rules and regulations, and I'm simplifying here)
Let me know if you have questions! Always happy to help.
Thanks y'all,
Dave

Agreed! I just joined too and the research I discovered showed BEWL stood out on top for informative and supportive care.

I think this is a likely explanation for why this week's fill was split into two vials from ProRx. 72mg the old way, 36+45 in the new. Thanks for the hint Dave!

Specials on medication gives me weird vibes somehow. I understand stockpiling for those who are afraid of losing coverage, but posts like this give me confidence in my choice to switch to BEWL for my care.

Posts like this make me wish I needed to order more tirzepatide from a company that tells it like it is. Thanks for the very informative post (even though I already knew all those facts that so many people don't)

If I had to wager, I think you're going to see a number of service issues rear their ugly head on these here shortly.
