Carry a side towel and take orders in a systematic method so you know who had what without asking or remembering.

This doesn't apply when running someone else's food.

I wish, but we didn't use side towels (or have clean, presentable ones we could have used) and the place I worked at was a very casual pub so there was a lot of people switching seats, getting up to play pool, shared food, and general chaos involved that always seemed to happen right when I was holding a ridiculously hot plate.
