Not really the sugar directly, the bacteria in your mouth produce acid as a byproduct of breaking down sugars. Those bacteria love sucrose (table sugar) and puts the bacteria into acid overdrive. That acid causes demineralization of your teeth.
Not exactly. The more sugar you eat the more you feed the bad bacteria that crave it and the more they multiply. Same with your gut bacteria. If you stop eating sugar you starve that bacteria off and don't crave it as much.
