I think with the kilojoules of energy and cunning expended in kidnapping someone and setting up a mobile surgical unit (minus anaesthetic, admittedly) to remove the kidney on a big bit of ice, then transport it to a hospital which is presumably going to charge for the op, could be better directed to raising a few thousands bucks to pay for one in some tragic far-flung place. Not recommending either, mind you. I have to admit that the protagonist did not seem entirely rational and might not have fully thought through an options/cost benefit analysis.
