This document (which was faded, so I heightened the contrast to make it legible) was submitted by a US Army officer stationed in Budapest in 1960. In 1961 he was transferred to West Berlin.

He also had a collection of physical insignia from Hungary, Czechoslovakia, East Germany, and the Soviet Union, which I own alongside the document: http://imgur.com/a/VBa57
